Atypical manifestation of parathyroid carcinoma with late-onset distant metastases

چکیده
Parathyroid carcinoma is an extremely rare endocrine malignancy that accounts for less than 1% of cases of primary hyperparathyroidism. We report a 44-year-old woman who presented with fatigue and diffuse bone pain. Laboratory findings revealed highly elevated serum calcium and parathyroid hormone (PTH) levels and a 4.5 × 3 × 2.5 cm cystic lesion in the lower pole of the right thyroid lobe that was shown histologically to be a parathyroid carcinoma. Ten years later, the patient developed brain and pulmonary metastases and recurrence of PTH-related hypercalcemia. Treatment of hypercalcemia along with localized radiotherapy and various chemotherapy regimens failed to induce a biochemical or radiological response. In conclusion, parathyroid carcinoma is a rare neoplasia that may develop metastases even after prolonged follow-up, for which there is no evidence-based treatment besides surgery. Different chemotherapeutic schemes did not prove to be of any benefit in our case highlighting the need for registering such patients to better understand tumor biology and develop specific treatment. Learning points Metastases can develop many years after parathyroid cancer diagnosis.Surgery is the only curative treatment for parathyroid carcinoma.Chemotherapy and radiotherapy prove to be ineffective in parathyroid cancer treatment.Patient registering is required in order to delineate underlining pathology and offer specific treatment.

Most of the patients at the time of diagnosis are young and the mean age is 40 year-old and occur with equal frequency in both sexes. In most series, serum calcium levels are ≽ 14 mg/dl and in half of them, a palpable cervical mass is evident. Clinical manifestations are maily due to hyperca...

Eccrine carcinoma is an extremely rare malignant skin cancer arising from eccrine sweat glands with a high metastatic potential. It mainly occurs in the elderly, with equal incidence in both sexes. It usually spreads to regional lymph nodes, with liver, lungs, and bones being the most common sites of distant metastasis. Because of tumor rarity, little is known about the value of F18- FDG PET/CT...
متن کاملZosteriform metastasis of ovarian carcinoma
Distant metastases are rare during the course of ovarian carcinoma. Ovarian epithelial adenocarcinomas rarely metastasize to the skin which is seen in less than 4% of patients. It usually presents as subcutaneous nodules and, less commonly, as inflammatory metastases which mimic cellulitis or viral or bacterial skin infections. The histology in most of the tumours is grade 1 or 2 serous p...

with poor prognosis, rapid growth, distant metastasis, early regional metastasis, and a high rate of recurrence. The parotid gland is the most common site of involvement, and the lungs and the bones are the most common sites of distant metastasis of SDC. Herein, we present a case of SDC of the parotid gland in a 62-year-old male patient with an unusual metastasis to the skin of the primary site...
